The symptomatic presence of self-loathing often pulls for a �countertransference wish or fantasy in which we seek to convince our patient to give up what can become his or her constant refrain of selfreproach. However, generally speaking the goal of our work lies in finding out why our patient feels this way about himself and perhaps, if we are fortunate, uncovering some of the unconscious determinants and functions that self-loathing might serve. I find that one of the most conspicuous features of working with selfreproach is how my experience of it changes over time with each patient.

Such shifts also occur in my erotic feelings—or lack thereof—or how enlivening or deadening it feels to be with a patient, as well as variations in sadness, fun, and excitement of all kinds. I was aware that James had limits on how much affection he could experience coming his way from his wife, his partner, or me. At home, when he felt that he and his wife were getting closer after a good weekend, he would often unconsciously end the weekend by doing things that annoyed her. For example, he would forget to mention to her something about an evening meeting for the upcoming week when it would have been just as easy to let her know in advance.

Instead, he felt that he should already know. Some of these feelings of embarrassment about vulnerability would come up in adult life. He was prone to feel critical of his friends when they would expose any naiveté or lack of knowledge in public. Traveling in a European country with a female friend was torturous because she would sometimes carry around a guidebook. Leonard was very sophisticated about this country and did not need such a guide. He was repulsed by her palpable demonstration of need and dependence even as he chastised himself for his reactions.
